About Ty hiraeth

Here at M&D Care we are passionate about providing support for individuals with complex needs, many of whom have experienced multiple community placement breakdowns and/or prolonged hospital admissions due to complexity and risk relating to their behaviours.

In early 2020 M&D Care introduced the flat-led model of support within a least restrictive environment by developing a new service Ynystawe Lodge. This service was specifically designed to enable individuals to have their own robust individual living space, whilst also offering them opportunities to develop tolerance and coping skills at their own pace, through the careful planned use of communal living areas.

Following on from the success of this flagship service and feedback received from individuals, families, and care teams and along with evidence of improved quality of life outcomes for the individual’s supported, M&D Care identified the need to develop a similar specialist bespoke service in Carmarthenshire. Ty Hiraeth has been developed with the same in-house support structure and has been built around individual needs and is designed to be adapted to meet the bespoke requirements of the person requiring support. Like Ynystawe Lodge, PBS practitioners will work full-time within Ty Hiraeth to ensure ongoing practice leadership and to embed PBS and Active Support into their everyday lives. Additional support will also be provided in-house through M&D Care’s specialist support services, including qualified learning disability nurses and PBM trainers.

This former Bible College has been painstakingly converted into a purpose built modern, spacious environment. Ty Hiraeth is a registered service but is different to a typical residential care home model. The accommodation and support are bespoke and adaptable to the complex and changing needs of the individuals requiring support.

Ty Hiraeth has been established to meet the needs of those who can present with the most complex challenges, including individuals with needs relating to Learning Disabilities, Autism and Mental Health. Our flat-led model of support offers the use of positive interventions within individually designed environments, to enable and support people who have very complex and challenging needs within the principles of least restrictive practice. The service provides communal living and facilities specific to high-risk environmental need.

Accommodation and Location

Ty Hiraeth is situated within Pen-y-groes, a small Carmarthenshire village. Located in a quiet location, however it is ideally situated in the heart of the local community with easy access to the motorway and town centre. The main building is modern and spacious, with wide corridors and open communal areas, with a large communal kitchen and separate life skills training kitchen. Accommodation comprises of two complexes, located across two levels. The ground floor has 6 self-contained flats, all with private gardens and the first floor has 3 self-contained flats. Each outdoor space has a built-in trampoline and astroturf area. Flats have their own bathrooms with living and bedroom areas. Furniture and fittings are robust and built to meet the required needs. All accommodation and communal areas on the ground floor are wheelchair accessible.

Ty Hiraeth Bungalow (opened October 2020)

There is a specially designed bungalow for two people that provides accommodation away from the main building whilst retaining use of the communal facilities available. Residents of the bungalow are supported within their own separate living and garden areas. This service provides adaptable and robust high need environments for people who find living and sharing personal space with others very difficult.
