At M&D Care, we value what the family members of the people we support have to say. We understand we are in a position of trust to provide quality care and support for your loved ones. Your feedback is extremely important to us as it is an opportunity for us to understand your views. This year is the first year the questionnaire has gone online which now means it is even easier to complete!

It is really important for us to capture a full understanding of how you feel about the quality of care and services M&D Care provide for the people we support.
Each section of the questionnaire covers a range of different topics from well-being, interactions with staff and management to the services and support provided. You can select an answer from a sliding scale from strongly agree to strongly disagree. There is no right or wrong answer, please be completely honest in your responses.
There is also an opportunity for each section of the questionnaire for you to provide your comments. We would really value you completing all of the comments sections to tell us your thoughts. We would find this information incredibly useful, as it lets us understand your points of view, suggestions or concerns. This allows us an opportunity to effect positive changes from the feedback we receive.
